Loan forgiveness FAQ for physicians

How much of my medical school debt can be forgiven through PSLF?

The entire remaining balance is forgiven tax-free after 120 qualifying payments. For physicians pursuing PSLF from residency start, this typically means $80,000–$200,000 forgiven depending on specialty salary and debt amount. Primary care physicians with lower attending salaries see the largest forgiveness amounts because their IDR payments are smaller throughout the 10-year window.

Is loan forgiveness taxable for doctors?

PSLF forgiveness is tax-free under current federal law. IDR forgiveness (after 20–25 years) is taxable — you will owe income tax on the forgiven amount in the year of forgiveness. NHSC and most state forgiveness programs are also tax-free at the federal level, though state tax treatment varies.

Can I stack NHSC forgiveness with PSLF?

Yes. If you work at an NHSC-approved site that is also a qualifying PSLF employer (most FQHCs and rural health clinics qualify), you can receive NHSC repayment assistance while simultaneously earning PSLF payments. NHSC payments don't count as PSLF payments themselves, but the years overlap.

What happens if PSLF changes or gets eliminated?

PSLF is established by federal statute (the College Cost Reduction and Access Act). Existing borrowers pursuing PSLF have historically been protected when changes occur. The safest approach is to continue pursuing PSLF if you qualify, while also modeling what aggressive payoff would cost — so you know your worst-case scenario.

Does loan forgiveness affect my credit?

PSLF forgiveness is reported as "paid in full" on your credit report. It does not negatively affect your credit. The discharged amount is not reported as income to credit bureaus. Taxable IDR forgiveness similarly does not impact credit — only your federal income tax return.
