Most residents don't know this.

Every qualifying payment made during residency or fellowship at a nonprofit hospital counts toward the 120 needed for PSLF. That's 3–7 years of low IBR payments that chip away at your requirement — before your attending salary even kicks in.

SAVE plan is dead (March 2026)

The 8th Circuit vacated SAVE. Borrowers were auto-moved to Standard Repayment. Switch to IBR immediately — it's the best IDR option now and PSLF-qualifying.

How to qualify during residency

Consolidate if needed

If you have FFEL or Perkins loans, consolidate into a Direct loan before your first payment. Do this early — consolidation resets your payment count.

Enroll in IBR

SAVE is gone as of March 2026. IBR is the current best IDR option. Payments are ~10% of discretionary income — low during residency, which is the point.

Submit PSLF Employment Certification (ECF)

Submit the ECF form every year — don't wait until payment 120. Annual submission catches errors early and builds a paper trail.

Verify your employer qualifies

Common questions

Do moonlighting payments count toward PSLF?

Only if your primary employer qualifies. Moonlighting income from a for-profit entity doesn't affect your PSLF status as long as your residency program is at a qualifying nonprofit hospital.

What if I switch from a nonprofit to a private practice?

Payments stop counting the moment you leave a qualifying employer. Payments already made still count — you don't lose them. You just pause accumulation until you return to a qualifying employer.

Can fellows count PSLF payments?

Yes. Fellowship programs at academic medical centers and nonprofit hospitals typically qualify. Submit an ECF during fellowship to confirm and start the clock.

Is PSLF worth it for high-earning specialties?
